Yep, saw it happen at Roebling at an autox event there maybe 10 years ago, kid in an Integra on street tires overcooked it, went off the track surface and into the ditch, car rolled over onto the driver side but fortunately not all the way over. It obviously ruined his day, hate to see that.

It mostly has nothing to do with speed, the incident I mentioned was probably only 30 or 40 miles per hour. When you put a slalom or offset maneuver on a 50 foot wide track surface it doesn't provide much room for correction before you are off the racing surface and into the dirt. They usually happen as a result of some inexperienced driver trying to be a hero and save the run rather than accepting their mistake and giving it up.
