If you buy a remote with an already cut key, you'll need an uncut blank which is less than 6 bucks from Hong Kong (I got mine in 5 days):

Looking at about $8 for a new blade in eBay, but you have to find a way of removing the old one, cut the new blade(don't know how much locksmiths will charge), and attach the new blade to the switchblade. I think eva02's price for $25 is good. Won't find less than that very often in eBay.

Removing the cut key is very easy. I managed to remove mine with a paper clip.
Most locksmiths will charge the same to cut a key, as long as it fits into the machine, which it should. I had no problem getting mine cut for $5.
Now, I ran a switch blade key on my '04, but never got it to program. i tried to follow the DIY procedure online, but never got it... so I can't speak as to what a deal would charge.

I"ll re-post your quote in the DIY: Code your own key / remote. I surely didn't think and try using a paper clip, thanks for sharing Vyndictive and below is how you can program it.

Got the Colour Coded key from Mazda as a pre-order present
Went to the dealer to Code it. I saw them done on the spot...
HEY ... In fact... YOU DON"T NEED TO GO THERE! YOU CAN DO IT YOURSELF!
Procedure to code key:
Require:
2 coded keys (not your service/valet key please)
1 Uncoded key (Your new key)
Your 8
Yourself with a working hand
Have you service/valet key ready. In case you F up
Steps:
1 - Put your 1st coded key. Turn on to standby mode/pre-ignition mode but not starting the engine. Wait for 4 seconds.
2 - Put your 2nd coded key. Turn on to standby mode/pre-ignition mode and wait for 4 seconds.
3 - Put your uncoded key in. Turn on to standby mode/pre-ignition mode and wait for 4 seconds.
4 - Test all the keys.
Should work. If you have F up the keys, use the service/valet key to drive up to the dealer.
Enjoy
P.S. Don't know what I am talking about still, the following thread may help
