I need to order clear corners...

Are they still $52 +$5 shipping with the bulbs? I was reading on a previous post that someone got them from you are this price.

I looked at the form, seems a bit confusing. I want to Paypal him for some clear corners. Do I have to still fill out the form and fax it? Says only to fax it for CC orders.
