Check clutch pedal welding points and bleed clutch line.

Did you read through the links I gave you?

We're starting with the usual or most likely causes and we need you to investigate and report back.

Was any work recently done on your car?
Miss any shifts on your trip?
Anything else seem different about your car? Squishy brakes maybe?
When was your tranny fluid swapped last?
When was your brake fluid swapped last?
When was the last time you replaced your clutch?
How many miles are on your car?
When you say the pedal is "loos"...do you mean right to left or up and down?
When you say it is difficult to get the car into first and reverse, do you grind or does it just not go into gear?

But how would anyone film/photograph something that is bolted to the top of tranny? You can barely see it from the top if you know what you're looking for and you can barely get a hand on it from the bottom.

Update: Wow...good find Darkbrew

The little nipple behind the oil filter is for the secondary slave cylinder used for the clutch pedal and is what needs to be bled.

Did you torque them to spec or just get them tight enough?
