another Ignition breakup/ Hesitation

another Ignition breakup/ Hesitation

History of car,

-Engine replaced at 85 000km
-Then replaced again at 91 000km
-Currently 128 000km
-NGK Spark plugs with about 2000km on them
-OEM Coils & NGK Wires just installed
-Cleaned SSV & VDI
-Checked for Vacuum leaks which are there are none
-Fuel pump assembly replaced with another fuel pump assembly

Modifications
-Turboxs Midpipe/Exhaust
-Aem Cold Air Intake VFAD was capped

CELs
-p0410
-p0420

THE PROBLEM

When accelerating the car will have ignition breakup, this tends to be more frequent with half throttle apose to WOT. Car starts fine cold and hot, idles at 850-900 relatively fine

Any Theorys inwhich I havent explored?

Also, ESS has been cleaned and so has the MAF. I cant even drive the car with fear of something major happening. I am running a factory tune

What do the spark plugs look like?

What are the AFR's when the ignition is breaking up?
