what do you mean, "you've been putting it in" Was it low? Did you draing the whole system or are you 'topping off'?

Are you completely filling the tank to the top while engine is cool?

You don't know what method was used to fill the car with coolant. A little early in the game to blame it on a hose or a waterpump.
Could have overfilled
Could be blocking airflow. (I saw Scrmn do this on an SB cruise)

IF the pressure cap is not working or sealing properly, pressure and coolant will bleed out of the overflow prematurely. This is always one of the first things to check.

You have to assume some level of intelligence when someone posts a question.

How about some more information
Year, model(MT, AT), mileage, driving style etc.

Worst case, your engine seals are ruptured.
Responses above are the good possibilities.

I have a feeling when he did the coolant change, or whatever, he just filled it up all the way to the top, then later he found out coolant came out, and again he added more to it, and of course it came out again.

Simple terms : Overfilling.
