But what does "ON 1 CCM" and "ON 2 CCM" mean?

Don't know what that is, but it isn't a Diagnostic Trouble Code that you would get from a malfuntion light. CCM stands for Comprehensive Component Monitor so I would guess that this information is referring to a scan of the PCM, but something other than a trouble code. Get a DTC code, a "P" number, and we can tell you what the trouble is related to.
