$500 debit card
I sent in the paper work saying I wanted to keep the car but have not received any confirmation whatsoever.
Should I be worried? I think I sent it in around the third week of September.
How many of you have received you debit cards?
Will they send me something I need to show my local service department to recieve free maintenance?

Yes, in addition to the debit card you should receive some mail that will contain an ID card that you can present to the service department to show you are under the free warranty deal. Even without the ID Card the technicians should be able to look you up in the system as your car should be flagged with a special service code to indicate your free warranty service. No, I haven't received mine either but this is our situation as I know it:
I received a phone call from Mazda about two weeks ago confirming that they had received my signed letter and they told me that the debit card would be sent out in the first week of October. Likewise we will also receive a free maintenance card to present to the dealer. If your card is due for a scheduled service before you receive your card you should call Mazda and ask them to setup the appointment. That way they can tell the dealer that the service is supposed to be free.

Ok, I just called 800-222-5500 and talked to a Rep. They looked up my VIN and they could see that my paperwork was received (I sent the paperwork 9/25). Just like Speed Racer mentioned, if you encounter a problem trying to submit your car for service before receiving mail confirmation and your ID Card, my Rep suggested to have the dealer/service department call the number above and the Reps should be able to straighten out the situation.

I sent my letter to keep the car on sept 2nd and I still have no paperwork or debit card from mazda
I called and they said they got my letter and everything is set but I havent got anything yet in the mail

Sounds like they're just a little slow in getting this stuff sent out.
I'll just have to be patient, but everytime I'm in Best Buy I keep telling myself, "just wait for the debit card". Its driving me crazy .

i sent mine in about 4 weeks ago and no mail yet...so far i've bought Blaupunkt Velocity 8" subs, amp, crossover, changed all my lights, clear corners, MP3 player, tints, rotary accents, side strakes so this $500 is prolly down to almost nothing...
