Waiver attached to the first post folks. Please sign via Adobe and please make sure to click "DONE SIGNING" so taht the signature gets merged into the document!

If this is to be notarized.

Why doesn't the document contain provisions for:
Name of Notary.
Commission Expires.
County of Residence
And the acknowledgement statement that the notary witnessed the signature.

(These are all concerns from my notary)

Thanks for that information. It seems that getting the form notarized isn't going to be required. Instead a digital signature via adobe is required.

I couldn't physically sign it. It would only allow me to do the digital signing (EG encryption key signing) not the physical handwritten signing.

When i try to do the handwritten signing via adobe it's grayed out.

it looks like you have the old form. I have a newer one on post 1 that will allow the signatures.

After signing click "Done Signing" and then click "Send Signed Document". Send it to

should work, I got 2 waivers already.
