I have not received the letter from Mazda yet, so I downloaded the form from a link off this forum. But what is the address I need to send the form to?

I called the RX-8 CS number (800-662-6779), but was on hold for over 30 minutes. I bet I can get an answer faster from this forum!

Ah... you may still want to call them. They noted in the computer that I had asked for the form and was going to be sending it out.

It took me about 45 minutes on hold today.
