This same thing happened to a friend's sister's fiance a few years ago.

I never use jack stands anymore. Local "almost" failing stories and friends experiences have convinced me.
I now use cribbing. Easy to build and WILL NOT FAIL PERIOD.
At the track I do have to use jack stands, but I am only under the car while the wheel is still on, the jack is still up and there are people with me. Also no heavy work, only adjusting my coil overs.

Maybe someones life will be saved since this sad and tragic story was shared.

Interesting, what do you use to crib the car with?

I work on heavy equipment, like construction, paving, and forestry and such, and we crib those machines with blocks of oak wood. We have jackstands as well, lifts, and whatnot, but we do use blocks of oak for cribbing.

I've personally had my car jacked up, four jackstands, and the jack, and it somehow ended up falling forward. When it did that, well, it crushed the jack in.


For those that just use a jack, you're pretty much relying on an o-ring, for the hydraulics on it to not fail.

But as far as jackstands go, I've been thinking about getting the kind with the pin, as I have a set of 4 of the ratchet style, I guess you could say, rated at 3 tons.
