Having worked 9 years in Car Rental I can say getting the smell out is very hard. Certain lower tar brands of cig's go away after time but Marlboro Red for example lasts for ever. Here is a LINK with some things you can try, a pro detailer might be the best bet.

Thanks for the suggestions and links, Lots of help. I am currently shampooing the carpet and floor mats as well as cleaning the headliner. Odor seems to have lessened already.

Febreeze extra strength has worked for me in the past. Whenever I get a rental that's been smoked in the first thing that enters the car is the extremely large bottle.
I haven't had any negative effects from using it. Has anyone ever heard of any?
Airing the car out after wetting it down with Febreeze is the best way I've found to pull the odors out.

Not trying to be so nonchalant......just still upset. She had an elderly(PC?) couple pull out in front of her on a four lane road, she moved over to the passing(L) lane and they continued on into her lane .....she didn't have enough distance to stop. Driving at about 40MPH. No airbag deployment. They are claiming neck injuries. Hope not to get sued. My daughter did have a witness so hope to get through this okay. We'll try to post photos.
