When the car is in nuetral and just idling if you can feel it vibrating "out of rhythm". You will hear it as well in the tailpipe as well. Let the car idle and go back there and see if you can pick out an off beat rhythm.
some of the fluctuating RPMs i think (when it's really cold anyway) is the engine warming itself up. I've noticed that when it's really cold (like zero or below) the car will rev to almost 3k on startup and gradually wind down. I do still understand the "rough idle" problem though.
